This is the Stephar - Minimal UI skin slightly modified to remove the auto attack button.
Select StepharNoAA in the options panel.
Skin only tested for 1920*1200. If you have a different resolution edit the line:

Where:
X= Screen resolution width - 1024(toolbar background image width) / 2
Y = Screen resolution height - 200(toolbar background image height)
